In Loving Memory of Forn Eugene Johnson, Jr.
Forn Eugene Johnson, Jr. was born on January 27, 1961, in Rome, Georgia, to the late Forn Johnson, Sr. and Pauline Johnson. He earned a degree in Accounting and Finance before relocating to California, where he obtained his Commercial Driver's License.
At an early age, Forn professed his faith in Jesus Christ and attended Pathway Missionary Baptist Church. He knew God as his Alpha and Omega and trusted Him wholeheartedly. His faith was steadfast and unwavering, serving as the foundation of his life.
Forn was a devoted father, brother, grandfather, uncle, friend, and partner who loved his family beyond measure. He was employed by the City of Indianapolis in the Department of Public Works.
In 1982, Forn married the love of his life, Jacqueline Yvette. Together they built a loving home centered on faith, family, and community. He dedicated much of his time to his wife and children and became a mentor to many young men. He instilled life skills, emphasized the importance of education, and invested quality time in the lives of his children and neighborhood youth. Family was always his priority, and he and Jacqueline Yvette enjoyed opening their home to family and friends for memorable gatherings.
Forn was known for being firm, disciplined, and true to his word. If he said it, he meant it. He strongly believed in education, leaving a lasting legacy, and building generational wealth for his family. In 2009, his family's journey changed with the passing of his beloved wife, Jacqueline Yvette. He faithfully honored the vows they shared until death parted them.
A lover of fellowship, Forn especially enjoyed playing Spades and Bid Whist. It was through his love of cards that he met his fiancée, Frances Allen, during the winter of 2009. They remained faithfully committed to one another throughout the years they shared together.
Forn was also a passionate sports fan who enjoyed watching and discussing nearly every sport. He loved watching the Food Network, experimenting with new recipes in the kitchen, listening to old-school music, and dancing whenever the opportunity arose. Spending time at the casino was one of his favorite pastimes.
On Wednesday, June 10, 2026, surrounded by the love of his family, Forn Eugene Johnson, Jr. peacefully transitioned from labor to reward. During his 65 years, 4 months, and 14 days of life, he touched countless hearts through his wisdom, generosity, strength, and unwavering love.
He was preceded in death by his parents, Forn Johnson, Sr. and Pauline Johnson; his beloved wife, Jacqueline Yvette Johnson; his brothers, Charles Johnson and Matthew Johnson; and his sisters, Minnie Leath and Mattie Williams.
Forn leaves to cherish his precious memory his sons, Ramond Donte Johnson and Delance Dewayne Johnson; his daughter, Jacqueline Renee Johnson; his devoted fiancée, Frances Allen; his sisters, Bobbie Frazier, Lena Johnson, and Fornia Thomas (Aaron); his beloved grandchildren, Michael, Dwight, Elijah, Idreese, Icesis, Isia, Delilah, and Dalani; along with a host of cherished nieces, nephews, lifelong friend, Eric Riggs; extended family members, and dear friends.
Though his earthly journey has ended, Forn's legacy of faith, love, wisdom, and devotion to family will continue to live on in the hearts of all who knew and loved him.
